I'm finding a little more of a consensus for Title Style: "16 Different Flavors of Hell." That format also stipulates that the first and last words be capitalized, regardless of length.

I just think it looks better and is easier to read. Running across a capital "A" in any title always just looks wrong to me.
Also, while the Wikipedia page on Capitalization offers no clear rule, I looked up The Beatles and Pink Floyd on Wikipedia and the song titles were, in fact, in Title Style: "Ticket to Ride," "A Day in the Life," "Careful with That Axe, Eugene" and "Set the Controls for the Heart of the Sun" being prime examples. That's as good an answer as I need.
